1. Acceptance of these terms
These Terms & Conditions (“Terms”) govern your use of daytondr.com (the “Site”), operated by Dayton Pain & Injury (“we,” “us,” or “our”). By using the Site, booking an appointment, or opting in to our communications, you agree to these Terms. If you don’t agree, please don’t use the Site.
2. Not medical advice
Content on the Site is for general information only and is not a substitute for professional medical advice, diagnosis or treatment. Reading the Site, submitting a form, or messaging us does not create a doctor–patient relationship. Always seek the advice of a qualified provider about your condition.
If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Don’t use the Site, forms, email or text messages for emergencies.
3. Appointments and cancellations
Appointment requests made online are not confirmed until our office confirms them. Please give at least 24 hours’ notice if you need to cancel or reschedule by calling (937) 910-0463. Same-day availability is not guaranteed.
4. Insurance, billing and claims
Statements on the Site about insurance, billing and costs, including “no out-of-pocket cost on qualifying claims,” are general descriptions. Coverage depends on your specific policy, the facts of your claim and applicable law. We will verify your benefits, but we do not guarantee coverage, payment by any third party, or the outcome of any insurance or legal claim. You remain responsible for charges not paid by a third party, as described in your patient financial agreement.
We are a healthcare provider, not a law firm, and we do not give legal advice.
5. Transportation
Free transportation is offered to eligible patients for scheduled appointments, subject to availability, service area and our transportation policies. We may decline or change transportation arrangements at our discretion.
6. Text message (SMS) terms
By providing your mobile number and opting in, you agree to receive text messages from Dayton Pain & Injury about appointments, reminders, follow-ups, billing and clinic updates. Message frequency varies. Message and data rates may apply. Reply STOP to cancel at any time; you’ll receive one final confirmation message. Reply HELP for help, or call (937) 910-0463. Carriers are not liable for delayed or undelivered messages. Consent is not a condition of receiving care. See our Privacy Policy for how we handle your information.
7. Use of the Site
You agree not to misuse the Site, including by attempting to gain unauthorized access, interfering with its operation, submitting false information, or using it for any unlawful purpose.

9. Patient testimonials
Testimonials reflect the experiences of individual patients. Results vary, and testimonials are not a guarantee of any particular outcome.
10. Third-party links
The Site links to third-party websites and services, such as Google Maps and our booking calendar. We don’t control and aren’t responsible for their content, policies or availability.
